These Terms and Conditions (“Terms”) govern your access to and use of OrgIQ Suite, LLC websites, pages, funnels, products, and services (collectively, the “Services”), including the Culture Health Scorecard page located at:

By accessing or using the Services, you agree to these Terms. If you do not agree, do not use the Services.

OrgIQ Suite may provide assessments, scorecards, resources, consultations, and other business services.

Unless explicitly stated otherwise in writing:

• Content is provided for general informational purposes and does not constitute legal, financial, HR, or other professional advice.

• You are responsible for how you use any information or recommendations.

4) User Submissions and Accuracy

If you submit information through a form (including contact details and scorecard responses), you agree that:

• The information you provide is accurate and current to the best of your knowledge

• You will not submit information you do not have the right to provide

• You understand we may contact you using the details you provide, consistent with your communication preferences and consents

5) Communication Consent

By submitting your contact information, you agree that OrgIQ Suite may contact you via email or phone for service related purposes. Marketing communications will be sent only when you opt in, and you can opt out at any time.

6) SMS Text Messaging Terms (A2P 10DLC Compliance)

If you opt in to SMS, OrgIQ Suite may send text messages to the mobile number you provided.

Message types may include

• Transactional messages related to your account, requests, appointments, orders, or services (for example appointment reminders, confirmations, service notifications)

• Marketing and promotional messages (for example special offers, discounts, and product or service updates), only when you expressly opt in

Message frequency
Message frequency may vary based on your interaction with OrgIQ Suite and the type of messages you opted in to receive.

Cost
Message and data rates may apply.

Opt out
You can opt out at any time by replying STOP. After you opt out, you may receive one final message confirming your opt out.

Consent

• SMS consent is not a condition of purchase.

• If you opted in to marketing messages, you may still receive transactional messages when you request services, unless you opt out of all SMS.

You agree not to use our SMS program or Services in a way that violates applicable laws or regulations, including laws relating to telemarketing, privacy, and consumer protection.

7) Privacy

9) Acceptable Use

You agree not to:

• Submit malware, spam, or abusive content

10) Third Party Links and Tools

The Services may include links to third party websites or tools. OrgIQ Suite is not responsible for third party content, policies, or practices. Your use of third party services is at your own risk and subject to their terms.

11) Disclaimers

The Services are provided on an “as is” and “as available” basis. To the fullest extent permitted by law, OrgIQ Suite disclaims warranties of any kind, whether express or implied, including implied warranties of merchantability, fitness for a particular purpose, and non infringement.

We do not guarantee specific outcomes or results from using the Services.

12) Limitation of Liability

To the fullest extent permitted by law, OrgIQ Suite will not be liable for any indirect, incidental, special, consequential, or punitive damages, or any loss of profits, revenue, data, or goodwill, arising from or related to your use of the Services.

To the fullest extent permitted by law, OrgIQ Suite’s total liability for any claim related to the Services will not exceed the amount you paid OrgIQ Suite for the Services giving rise to the claim, if any, during the three months preceding the event giving rise to the claim.

13) Indemnification

You agree to defend, indemnify, and hold harmless OrgIQ Suite and its owners, employees, contractors, and agents from any claims, liabilities, damages, losses, and expenses (including reasonable attorneys’ fees) arising out of or related to your use of the Services, your violation of these Terms, or your violation of any law or third party rights.

14) Changes to the Services or Terms

We may update the Services and these Terms from time to time. When we do, we will update the “Last updated” date above. Your continued use of the Services after changes are posted means you accept the revised Terms.

15) Governing Law

These Terms are governed by the laws of the State of Missouri, without regard to conflict of laws or principles.
